Human Support Services is seeking to add a full-time Case Manager, to our premier team of mental health professionals to provide mental health treatment in Monroe County, IL. This position will work in our supported residential housing.

Job Duties Include:

•Case Management

•Advocacy, referral and linkage to Community Support Services

•Community Education

•Documentation of clinical services

•Utilize a variety of therapeutic interventions to address clinical needs.

•Complete required data entry and paperwork in accordance with program and agency policies and procedures, including but not limited to opening requirements, assessments, treatment/recovery plans, progress notes, treatment authorizations and authorization of medical necessity.

Requirements

* Minimum of a Bachelor's degree in a Human Services field (Psychology, Social Work, Counseling, etc.)

* One year relevant job experience in case management preferred.

* Must be willing to work in a multidisciplinary team environment.

* Must have a valid driver’s license and be insurable for auto coverage.
